چکیده

Increasingly aggressive treatment protocols are being used in the hope of improving patient survival rates. These regimens, however, can lead to a wide spectrum of complications owing to the overlap between “effective” and “neurotoxic” doses of radiation. Differentiating tumor recurrence from treatmentinduced change is a clinical and radiological challenge. Researchers have suggested three mechanisms to explain radiationinduced neurotoxic effects in the brain: • Direct cellular damage. Radiation generates reactive oxygen species that damage cellular DNA, eventually leading to the induction of cellular apoptosis. Tumor cells with a high mitotic rate are affected the most because they are less able than normal cells to instigate DNA repair.

Relative value of magnetic resonance spectroscopy, magnetic resonance perfusion, and 2-((18)F) fluoro-2-deoxy-D-glucose positron emission tomography for detection of recurrence or grade increase in gliomas

In a consecutive series of 26 previously operated patients diagnosed with cerebral glioma, magnetic resonance spectroscopy (MRS), 2-((18)F) fluoro-2-deoxy-D-glucose positron emission tomography (FDG-PET), and perfusion MRI (MRP), were performed at follow-up to distinguish recurrence from radiation necrosis, and to identify tumour upgrading. Discrepancy between techniques was observed in 9 cases...

Differentiation of local tumor recurrence from radiation-induced changes after stereotactic radiosurgery for treatment of brain metastasis: case report and review of the literature

BACKGROUND Structural follow-up magnetic resonance imaging (MRI) after stereotactic radiosurgery (SRS) for brain metastases frequently displays local changes in the area of applied irradiation, which are often difficult to interpret (e.g., local tumor recurrence, radiation-induced changes). The use of stereotactic biopsy for histological assessment of these changes has a high diagnostic accurac...

Integrated Systems and Technologies Hyperpolarized C Spectroscopy Detects Early Changes in Tumor Vasculature and Metabolism after VEGF Neutralization

No clinically validated biomarkers exist to image tumor responses to antiangiogenic therapy. Here, we report the utility of hyperpolarized C magnetic resonance spectroscopy (MRS) to detect the early effects of anti-VEGF therapy. In two colorectal cancer xenograft models, displaying differential sensitivity to VEGF blockade, we compared hyperpolarized MRS with measurements of tumor perfusion usi...

Differentiation of Radiation-Injuries and Tumor Recurrence Using Perfusion-weighted imaging

The differentiation of progressive or recurrent brain tumor from radiation injury after radiotherapy is difficult using conventional MR imagings(MRI) [1]. Accurate diagnosis of tumor recurrent or radiation injury is critical to determining therapy [2]. Perfusion-sensitive contrast-enhanced MR imaging(PWI), which has made it possible to obtain measurements of vascularity within brain lesions, ma...

Radionecrosis of malignant glioma and cerebral metastasis: a diagnostic challenge in MRI.

Detecting a new area of contrast-enhancement at MRI after irradiation of malignant brain tumor arises the problem of differential diagnosis between tumor recurrence and radiation necrosis induced by the treatment. The challenge for imaging is to distinguish the two diagnoses given: the prognostic and therapeutic issues. Various criteria have been proposed in the literature based on morphologica...
